Dear Colleagues,
In an effort to identify, prioritize and move forward with planning for vaccination availability for all MCPS staff, staff are asked to complete the questionnaire below. This information will allow the Employee and Retiree Services Center and MCPS vaccination partners to appropriately prioritize and acquire vaccinations for staff returning to in-person work.
Important note: If you have received a vaccine, please return to the questionnaire and report your updated status.

In addition, more MCPS employees are receiving appointment invitations from the Montgomery County Department of Health and Human Services (DHHS). Employees will get an email from either Johns Hopkins Medicine or DHHS, not both.

The email invitation from DHHS is individualized to MCPS employee email addresses. Please do not share or forward this link. If you share the link, your appointment could be adversely impacted. The DHHS email will come from a Montgomerycounty.gov email address.

The email from Johns Hopkins will require employees to create a “My Chart” account. This information will be included in the subject line of the email.
